Position: Clinical Dietitian (Home Based Primary Care)

Summary

Employees in this assignment serve as a Clinical Dietitian providing Home Based Primary Care (HBPC) within the Clinical Section of Nutrition and Food Service. Clinical Dietitians providing HBPC coordinate nutritional care of Veterans with other health care providers – the Veteran, the Veteran’s caregiver, and the community.

Qualifications Basic Requirements
  • United States Citizenship: be a citizen of the United States (U.S.)

  • Registration: all applicants must hold the RDN credential from the Commission on Dietetic Registration (CDR), the credentialing agency of the Academy of Nutrition and Dietetics.
  • Note:

    Applicants who hold the RDN credential have met the education requirement (e.g. bachelor’s degree or higher) of CDR; therefore this occupation is considered professional and scientific with a positive education requirement and the procedures in Part I – Chapter 4 of this Handbook will be followed when applying Veteran’s preference.

  • May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
  • Grade Determinations: in addition to the basic requirements you must meet the following grade requirements:
    Dietitian (Clinical) – GS‑11, at least one year of experience as an RDN equivalent to the next lower grade.
  • Specialized experience includes – but is not limited to: conduct nutrition assessments using the NCP; apply evidence‑based nutrition practice guidelines; interpret research and utilize whole health principles to develop patient‑centered goals and improve outcomes; maintain accurate record‑keeping of daily clinical activities and patient encounters using approved systems; utilize virtual care technology to improve access to care; and demonstrate knowledge, skills and abilities.
  • In addition to meeting the experience or educational requirements for this grade level, the candidate must fully demonstrate the following KSAs: skill in delivering patient care using the NCP (assessment, diagnosis, intervention, monitoring and evaluation); skill in interpreting research to apply evidence‑based nutrition practices to improve patient outcomes; skill in developing patient‑centered goals; skill in analyzing data for performance improvement studies;

    skill in collaborating with stakeholders; ability to comply with various accrediting, regulatory and agency authorities.

Duties
  • Conduct comprehensive nutrition assessments using the Nutritional Care Process (NCP), including nutrition‑focused physical examinations, to establish a nutrition diagnosis and determine risk and/or degree of malnutrition.
  • Develop and implement Medical Nutrition Therapy (MNT) to address the nutrition diagnosis; evaluate and monitor the effectiveness and outcomes of MNT interventions.
  • Modify the plan as needed to achieve positive patient outcomes.
  • Calculate nutrient requirements and necessary diet restrictions or food and drug interactions, tailoring the diet prescription to each individual patient.
  • Document MNT in the medical record using the ADIME format.
  • Function independently and accurately when record‑keeping.
  • Identify patients in need of nutritional support and recommend the appropriate mode of support; communicate appropriately with the health care team.
  • Actively participate in formal and informal interdisciplinary team meetings.
  • Assess educational needs and provide nutrition education and counseling for individuals and groups that is tailored to the Veteran(s).
