As a full‑time therapist at Center Well Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you’ll empower individuals to overcome limitations and enjoy a better quality of life.
Patients must reside in Washington and Ozaukee Counties, Wisconsin. This is a flexible, territory‑based role that is not remote and offers a sign‑on and relocation bonus.

- Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function.
- Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician’s plan of treatment and the patient team’s goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
- Use your skills to make an impact.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- One year of experience as a Physical Therapist
- Pay per visit/unit: $59.00 – $83.00
- Base pay: $93,000 – $128,000 per year
40 hours per week
Additional InformationTB Statement:
This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement:
This role is part of Humana’s driver safety program and therefore requires an individual to have a valid state driver’s license and to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
Travel:
While this is a remote position, occasional travel to Humana’s offices for training or meetings may be required.
