Trinity Health At Home in Fresno is seeking an Occupational Therapist to provide high-quality, home-based OT services in clients’ homes under the physician’s plan of care. You will help clients achieve independence and improved quality of life through therapy, education, and care coordination.
The OT supports caregivers, documents in the EHR, and collaborates with an interdisciplinary team. A CA OT license, NBCOT, and valid driver’s license are required; home health experience is preferred.
