Job Summary & Responsibilities

We are looking for a Clinical Assessor to join our team. The role is based in Anson and Union County, North Carolina, and involves a hybrid work model: working from a home office while traveling up to 60 miles to assessment locations in the region.

  • Conduct needs‑based eligibility determinations under the Personal Care Services (PCS) program for NC Medicaid beneficiaries requiring assistance with ADLs.
  • Perform assessments for the Community Alternatives (CAP) Program, identifying cost‑neutral community‑based options for beneficiaries at risk of institutionalization.
  • Complete level‑of‑care (LOC) assessments to help individuals remain in or return to home or community settings.
  • Work independently with limited supervision, coordinating with other staff, healthcare professionals, patients, and families.
Qualifications Required
  • Registered Nurse or Licensed Clinical Social Worker (LCSW) license in North Carolina.
  • Minimum of two years nursing experience.
  • Willingness to travel up to a 60‑mile radius.
Preferred
  • Experience with community‑based individuals requiring assistance with ADLs and IADLs.
  • Experience conducting PCS or HCBS waiver assessments.
  • 2+ years of home healthcare or case‑management experience in the health or medical field.
  • Knowledge of North Carolina Medicaid Clinical Coverage Policy (3L and 3K series), PCS program provider manual, and related standards.
  • Proficient with Microsoft Office (Excel, Word, Outlook) and relevant web‑based software.
  • Strong communication, detail orientation, and independent work skills.

Pay Range

USD $77,000 – $80,000 per year.
