United Health Group seeks a Community Health Worker – Housing Liaison to connect Medicaid members with essential care, conduct home and social assessments, and provide member education. You will engage primarily via telephone in a team-based structure, addressing social determinants such as housing and food access.
Typical schedule is Monday–Friday, 8–5 EST, with potential telecommute for VA residents. Local travel up to 25% and collaboration with care teams are required to improve member health
