Stroke Association in the North Lincolnshire area is seeking a Stroke Support Coordinator to identify needs of stroke survivors and carers, and to deliver tailored support across the pathway.
The role requires extensive home visits and travel across the region, with part-time hours and funded contract duration. You will work closely with the Stroke Service Delivery Coach to provide a person-centred service and to promote awareness of stroke and its risk factors.
