Qualified Mental Health Professional

POSITION

DESCRIPTION:

Under the direction and supervision of the Clinical Supervisor and Program Director, home-based counselors will deliver direct-care services to the clients and their families utilizing a community-based treatment modality.

IMMEDIATE SUPERVISOR:
Clinical/Program Director

DUTIES AND RESPONSIBILITIES
  • Provides services on an intensive basis utilizing short-term, solution focused treatment strategies to clients and families
  • Facilitates care coordination with other service providers to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports
  • Prepares written service plans and treatment update summaries, incorporating input from clients, other service providers, wraparound team members to identify problem areas and needs, service objectives, and intervention strategies
  • Provides direct clinical services to clients, including individual and family interventions
  • Responds to crisis situations with twenty-four hour a day availability
  • Completes progress reports in reference to service delivery within 48 hours
  • Utilizes Lauris (Electronic Medical Record system) and other web-based software for all clinical documentation, record keeping, and coordination.
  • Participates in child specific Family Assessment and Planning Team (FAPT) and other interdisciplinary, diagnostic, or planning meetings
  • Completes 20 hours per year of professional training provided by Integrated Health Services and outside providers.
  • Pass and complete DBHDS, DMAS, and Integrated Health Services required trainings on an ongoing annual basis to include but not limited to: HIPAA, Bloodborne Pathogens, TOVA, CPR, First Aid, Person Centered Practices, Human Rights, Mandated Reporting, and Emergency Preparedness.
  • Links clients with external programs or services, such as: health services, recreational activities, child care services, financial resources, housing placement, employment resources, child care, AA/NA groups, transportation resources, and others
  • Attends court hearings and probationary meetings when families are court-involved and presents testimony and treatment recommendations as requested by attorneys, referral source, the court, or clients
  • Meets clinical supervisor 2x per month for service delivery and collateral contact review, chart audits, case staffings, individual training, self-care check-in, and employee development planning
  • Attends bi-weekly staff meetings, group supervisions, and/or trainings
  • Prepares written Discharge Summary and Plan
RESIDENT IN TRAINING FOR LPC or LCSW LICENSURE

shall provide all of the above. In addition they will prepare written diagnostic assessments, assessing clients from multiple perspectives, including family functioning, psychological/emotional, social, academic, medical, behavioral, substance abuse, skills, and strengths, barriers to economic self-sufficiency, treatment history, and others and utilizing various assessment tools. The resident will also provide family therapy to clients. All of this will be done under the supervision of an approved supervisor.

IHS provides supervision opportunities to full time staff after the successful completion of probationary period. These opportunities include supervision for certification such as CSAC, CSOTP, and for residents in counseling seeking licensure for the LPC. IHS offers these benefits free of charge or contract, contingent on maintaining full-time employment and satisfactory performance review.

EDUCATION AND EXPERIENCE

A Bachelor’s Degree in Social Work, Psychology, Counseling, or a related field from an accredited college or university and one year of experience providing mental health treatment to troubled children and families is REQUIRED. Preferred Master’s Degree in Social Work, Psychology, Counseling or related human services field from an accredited college or university with one year of experience in the mental health field working with children and/or adolescents.

Must meet the criteria to be a Qualified Mental Health Professional (QMHP).
