Equip is seeking a Caregiver Coach to join a fully virtual care team. You will deliver skills-based sessions to caregivers, help families engage with care plans, and support patient progress within a multidisciplinary setting.
You will work remotely, conducting sessions and outreach while maintaining calendars and documentation in line with Equip policies. The role requires 33 hours of direct care weekly and two evenings until 8pm, with competitive compensation and extensive benefits.
