Overview Sentara Health is hiring a Full Time Care Coordinator for the South Central Richmond, VA area!

Hours:

Full Time (40 hrs/wk)

Shift: Day (8am-5pm)
This position requires travel for in person face-to-face assessments in around the south Central Richmond
- Dinwiddie, Petersburg, Prince George, South Hill . Days without assessments scheduled employee can work remotely.

Applicants must be located in the south Central Richmond Area of VA Education

Must possess a Bachelor's degree in Health & Human Services or one of the following related Fields:

Art Therapy Behavioral Sciences Child Development Cognitive Sciences Community Mental Health Counseling (MH, Vocational, Pastoral, etc.)Counselor Education Early Childhood Development Educational Psychology Gerontology Healthcare  Administration Human Development Human Services Marriage and Family Therapy Music Therapy Nursing Pharmacy Psychiatric  Rehabilitation Psychology Rehabilitation  Counseling Social Work Sociology Special  Education Speech Therapy Therapeutic Recreation Vocational Rehabilitation Certification/Licensure No specific certification or licensure requirements

Experience1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience (Required)
Experience supporting individuals with Intellectual and Developmental Disabilities (/DD) and waiver services (Preferred)
Experience working with individuals affected by substance use disorders and coordinating treatment or recovery support services (Preferred)
Experience advocating for patients, clients, or vulnerable populations and assisting with care transitions and service navigation (Preferred)
