On the United Healthcare team, you will serve as the primary care manager for members with complex medical or behavioral needs who are not managed by clinicians on the pod. The Community Health Worker (CHW) locates and engages members in the community, develops individualized care plans, identifies and overcomes barriers—including social determinants of health—and works collaboratively with the interdisciplinary pod to advance member-centered interventions.

Working

Schedule

Monday through Friday, 8:00 am – 5:00 pm. The role is field‑based with a home‑based office; you will work from home when not in the field.

Primary Responsibilities
  • Partner with the clinical care management team to manage complex members in the community.
  • Locate and engage members within the community.
  • Conduct face‑to‑face or telephone screenings to assess medical, behavioral, functional, cognitive, and psychosocial needs.
  • Research, locate, and refer community resources to address member‑specific care or socioeconomic needs; follow up to confirm services are received.
  • Perform targeted activities and provide education to support HEDIS/STAR gap closure, including scheduling, reminders, and verification of appointments.
  • Proactively engage members to manage their care, provide education, and coach self‑management of needs and health promotion as outlined in the care plan.
  • Collaborate with internal care teams, community partners, and providers.
  • Consult with the clinical team based on defined triggers.
  • Support identification and coordination of Medicaid benefits to meet care needs.
  • Assist members in developing personal wellness or crisis plans.
Required Qualifications
  • High School Diploma or GED.
  • 3+ years of experience working within the Community Health setting in a healthcare role.
  • 2+ years of experience working within the local community with intermediate knowledge of local culture and resources.
  • Access to reliable transportation to travel to client sites within a designated area.
  • Intermediate proficiency with Microsoft applications.
  • Ability to travel up to 10% of the time for the role.
  • Access to a designated quiet workspace in the home with high‑speed internet and secure handling of Protected Health Information.
Preferred Qualifications
  • Bachelor’s Degree (or higher) in Social Work or related field.
  • LPN (Licensed Practical Nurse) Licensure or CNA/HHA.
  • Bilingual in Spanish.
  • Community Health Worker (CHW) Accreditation.
  • Experience working in team‑based care.
  • Experience working in Managed Care.
  • Lived experience managing or overcoming a chronic health, behavioral health, or substance use condition.
  • Telephonic case management or customer service experience.
  • Demonstrated knowledge of Medicare and Medicaid benefits.

  • Transition easily from office to field locations multiple times a day.
  • Reliable transportation for client visits across varied distances and terrains.
  • Ability to carry necessary equipment such as laptop, stethoscope, and blood pressure cuff.
Pay and Benefits

Hourly pay ranges from $20.00 to $36.00 per hour based on local labor markets, education, experience, and certifications.
