General SummaryThe Medication Reconciliation Technician is a certified pharmacy technician that will function primarily within the medication reconciliation process. The Medication Reconciliation Technician will review the medication list of newly admitted patients, make changes to the medication profile including but not limited to clarifications of dose, dosage form, medication directions, etc. An accurate list of changes will be presented to the Medication Reconciliation Pharmacist for review.
Responsibilities- Perform a patient medication history; document the history accurately and completely. Actively include the patient or the patient's family, patient's pharmacy, patient's physicians, patient's prescription bottles to ensure proper identification of doses and medications, to include OTCs, herbals/supplements and/or query software programs to resolve all discrepancies in a timely manner. Number of completed patient interviews based on i-vents will be consistent with colleagues on an annual basis.
- Determine medications that may be needed to be brought from home based on formulary status, therapeutic interchanges and availability of medications. Non-formulary medications will be brought to the attention of the Pharmacist.
- Compilation and clarification of medication related issues in the medical record so that the attending physician /and or designee can review and reconcile the list in the appropriate software.
- Present a complete and accurate admission list to Medication Reconciliation pharmacist for review. Patients at highest risk for readmission and those with polypharmacy should be the focus and consistent with colleagues (i.e. Technician should not only take patients with 0-5 meds and not take any patients with greater than 11 meds). Errors within the Pharmacy Technicians work that is presented to the Pharmacist for review should be minimal.
- Document, review and submit appropriate Performance Improvement metrics to the Leadership Team using the interventions function within the current Electronic Health Record.
- All other duties as assigned, within the scope and range of job responsibilities.

High School Diploma or GED - Credential(s): CPHT within 12 months of hire, if not already obtained

One year experience as a pharmacy technician or equivalent experience (such as Pharmacy School Student)
