Cardiac Care Alliance in Dallas is hiring a full-time, 100% remote Care Coordinator to support medically complex CHF patients. You will conduct telephonic outreach, maintain caseloads, and document care plans following CMS PCM, CCM, and TCM guidelines.
Work closely with RNs and Cardiologists to coordinate transitions, address social determinants of health, and ensure timely follow-ups and accurate documentation in the care management platform while protecting PHI and HIPAA compliance.
