Location: Chaparral
Addus Home Care in the United States is seeking a full-time Home Health Weekend Intake Coordinator to support patients and families by coordinating care services that help individuals recover, manage chronic illness, and remain independent at home.
Hours are Friday through Monday, 8am–7pm MST, with remote work. You’ll receive referrals, explain benefits, coordinate initial nursing assessments, maintain records, and support admissions staff. Benefits include paid holidays, PTO, and retirement plan.
