What You’ll Do:
- Provide one-on‑one support to individuals with disabilities as they explore, secure, and succeed in competitive, integrated employment.
- Meet clients in their homes and community to build trust and connection.
- Conduct career assessments that focus on individual strengths, skills, and interests.
- Help develop or revise resumes, practice interviews, and identify job leads.
- Support onboarding and training with new employers.
- Provide ongoing coaching to promote job retention and personal growth.
- Attend team meetings, write progress reports, and maintain clear communication.
- Create positive, lasting impact—one job, one goal, one success at a time.
- Work hybrid with up to two days per week remote based on service needs.
- Mileage for work‑related travel is reimbursed.

QualificationsWhat You Bring to the Table:
- Required (Must‑Have)
- Must meet requirements of a Designated Coordinator.
- High school diploma or GED plus at least 4 years of experience providing direct care or support to individuals with disabilities or older adults OR
- Associate degree in human services, education, or a related field plus at least 2 years of experience providing direct care or support to individuals with disabilities or older adults OR
- Bachelor’s degree in human services, education, or a related field plus at least 1 year of experience providing direct care or support to individuals with disabilities or older adults.
- Valid driver’s license and proof of insurance.
- Must be at least 18 years of age.
- Physical Requirements
- Ability to stand, walk, bend, or lift.
- Skills & Abilities
- Strong written, verbal, and interpersonal communication skills.
- Ability to document care, follow support plans, and collaborate with internal and external team members.
- Experience teaching, coaching, or mentoring individuals in employment‑related skills.
- Knowledge of the job market and hiring process.
- Demonstrates integrity by maintaining confidentiality and following organizational policies.
- Highly organized, flexible, and able to manage multiple tasks or unexpected demands professionally.
- Fluency in ASL, Spanish, Hmong, Somali, or Oromo preferred.
- Proficient with using technology for documentation.
