* The Palliative Care Advance Practice Clinician delivers advanced-level, patient-centered care focused on symptom relief, emotional support, and improved quality of life for individuals living with serious, chronic, or life-limiting illnesses. They collaborate closely with interdisciplinary healthcare teams and families to ensure holistic, compassionate care is provided. In addition, they evaluate patients and recommend ongoing plans for longitudinal care teams to support continuity and quality of care throughout the patient's journey

* Primary Responsibilities:

*
* + Deliver palliative-focused direct patient care 100% of the time, ensuring compassionate, evidence-based practice

+ Conduct primarily palliative care telehealth visits and follow-up appointments for members with complex needs, including symptom management and medical decision-making

+ Holistic Patient Assessment:
Complete comprehensive evaluations addressing physical, emotional, cognitive, psychosocial and spiritual needs to create individualized care plans

+ Interdisciplinary

Collaboration:

Partner with interdisciplinary palliative care team members, internal longitudinal care teams and community providers - such as physicians, nurses, social workers, and therapists - to ensure coordinated, seamless care

+ Symptom & Pain Management:
Prescribe and adjust pharmacologic and non-pharmacologic treatments for pain, nausea, dyspnea, fatigue, and other distressing symptoms, including deprescribing when appropriate

+ Advanced Care Planning:
Lead goals-of-care discussions, assist patients and families in defining care preferences, and support completion of advance directives and end-of-life decisions, as well as confirming and completing orders pertaining to life sustaining treatments, when indicated

+ Patient & Family

Education:

Provide clear, empathetic education on disease processes, symptom management strategies, available services, and treatment options

+ Documentation & Quality Assurance:
Maintain accurate, timely clinical documentation; participate in quality improvement initiatives; and stay current with best practices in palliative care

* Required Qualifications:

*
* + Currently licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of practice

+ 2+ years of experience in palliative care, hospice, oncology, geriatrics, or a related field

+ Clinical Competency in Advance Care Planning and Serious Illness Conversations

+ Solid Primary Palliative Care Skills and Comfort with Tertiary/Specialized Palliative Care Skills

+ Proven ability to clearly articulate patient prognosis to people at all levels of health literacy, including patients, families, and community agencies/providers

+ Proven solid ability to manage complex caseloads, care plans, and documentation amidst fast-paced settings

* Preferred Qualifications:

* + Certification in Palliative or Hospice Care (Advanced Certified Hospice and Palliative Nurse (ACHPN))

+ Additional palliative care certifications (e.g., CAPC, Vital Talks designation, etc.)

+

Experience with hospice care, inpatient palliative care units, or related settings

Compensation for this specialty generally ranges from $125,000 - $175,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
