Position: OP Care Coordinator (McKeesport)
The Outpatient Care Coordinator provides initial comprehensive needs assessment, registration of clients requesting services and completion of initial paperwork required for clients to begin outpatient treatment at Western Psychiatric Ambulatory Programs. Additional responsibilities include provision of case management services including assessment, service linkage, resource development, coordination of all services received, and advocacy.

This position has a flexible schedule with no weekends or evenings and the ability to work from home one day a week once fully trained

** Responsibilities:*
* * Conduct comprehensive client assessments to identify mental health, case management, housing, medical, legal, substance use, financial, and social support needs.

* Apply recovery-oriented principles to promote client independence and self-management.

* Coordinate referrals, service linkage, and access to community, agency, and county resources.

* Provide case management services, monitor client progress, and coordinate care with treatment team members and community providers.

* Assist with involuntary commitment processes, including paperwork, transportation, and emergency resource activation.

* Facilitate hospital discharge planning and ensure continuity of care between providers and services.

* Complete intake assessments, develop provisional DSM diagnoses, and gather information necessary for ongoing treatment.

* Manage new client referrals and administrative intake processes while addressing clinical concerns with leadership and clinical staff.

* Monitor client engagement and attendance, intervening to address barriers to treatment participation.

* Answer clinic phone calls, provide information, connect potential clients to services, and support community referrals.

* Advocate for client and family choice, participation, and education throughout the recovery process.

* Maintain accurate, complete, and timely documentation, including case records, progress notes, referrals, intake forms, and county commitment paperwork.

* Track and maintain intake data, reports, statistics, and other required records.

* Obtain insurance authorizations and ensure compliance with payer and referral requirements.

* Participate in treatment team meetings and collaborate with multidisciplinary staff to support client care.

* Bachelor's Degree in Human Services or Business related fields.

Licensure, Certifications, and Clearances:

* Cardiopulmonary Resuscitation (CPR)

* Comprehensive Crisis Management (CCMC)

* Act 31 Child Abuse Reporting with renewal

* Act 33 with renewal

* Act 34 with renewal

* Act 73 FBI Clearance with renewal
