Job Overview
Position: DDP Targeted Case Manager (14289) )
Department:
Montana Department of Public Health and Human Services (DPHHS), Behavioral Health & Developmental Disabilities Division.
Purpose:
The Developmental Disabilities Program procures and supports community‑based services for people with developmental disabilities and/or their families.
- Conduct a comprehensive assessment and periodic reassessment of individual needs, and develop and periodically revise the plan of care.
- Refer the member to and coordinate related activities to help them obtain needed services.
- Monitor and follow‑up on the effectiveness of services and ensure the quality of the plan of care.
- Maintain all required documentation and reporting as mandated by DPHHS.
- Above average ability to follow written and verbal directions.
- Ability to communicate orally and in writing.
- Above average ability to recognize, analyze, and solve complex problems/dilemmas.
- Ability to facilitate, negotiate and mediate problems and promote team consensus.
- Bachelor’s degree in social work or a related field from an accredited college.
- Minimum of two years of experience in human services, with at least one year in the field of developmental disabilities. If lacking one year in developmental disabilities, you must complete at least 40 hours of training in service delivery to persons with developmental disabilities within three months of hire or designation as a case manager.
Salary: $28.55 per hour.
Telework eligibility:
Partial telework within the state of Montana; conditions provided by supervisor.
