This full‑time PT position is based in Milwaukee, Wisconsin. The role is not remote and relocation assistance is available.
Role SummaryAs a full‑time therapist at Center Well Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence from the comfort of their homes. Deliver personalized care focused on functional improvement.
Key Responsibilities- Plan and administer prescribed physical therapy treatment for patients in their homes.
- Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician’s Plan of Treatment and patient goals.
- Provide PTAs and Home Health Aides with written care instructions and oversee performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family; submit required evaluations, plans, progress notes, and discharge summaries.
- Degree from an accredited Physical Therapy Program (approved by the APTA).
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- One year of experience as a Physical Therapist.
- $59.00 - $83.00 per visit/unit.
- $93,000 - $128,000 per year base pay.
Patients in Washington and Ozaukee Counties, Wisconsin.
Travel and Work RequirementsThis role requires occasional travel to Humana’s offices for training or meetings. A valid state driver’s license is required and drivers must maintain personal vehicle liability insurance. Home or hybrid home/office employees must provide suitable internet service (minimum 25 Mbps download, 10 Mbps upload).
