We are looking for an In Patient Care Pharmacist, Specialty to join our Specialty Operations team. The role is ideal for an autonomous, highly skilled pharmacist who focuses on the accurate, safe, and efficient dispensing of prescriptions and is passionate about evaluating and improving internal processes.

Job DescriptionAs part of a multi‑state Specialty Operations team, you will develop and refine SOPs, participate in cross‑functional conversations, and carry out Fuze Rx’s Patient Management Program in compliance with URAC, ACHC, and VIPPS standards.
Primary Responsibilities- Provide consultations via text, app messaging, email, or phone.
- Perform initial assessments by reviewing chart notes to determine prescribing appropriateness.
- Conduct clinical check‑ins to monitor adherence, efficacy, adverse reactions, and drug interactions.
- Identify patients who need clinical interventions and collaborate with providers to offer therapeutic recommendations that improve outcomes.
- Ensure transition of care by transferring out prescriptions to appropriate pharmacies when Alto cannot fulfill a prescription.
- Communicate prescription statuses to patients and providers.
- Maintain an in‑depth understanding of pharmacy billing (Medicaid, Medicare, ADAP, commercial insurance, foundation assistance) to troubleshoot patient cost barriers.
- Proactively identify and communicate scheduling/delivery concerns.
- Develop and provide drug information materials to enhance Specialty Operations performance.
- Stay abreast of regulatory and industry standards, initiate appropriate actions, and communicate trends to applicable personnel.
- Work interactively with local and dispersed teams to achieve business and compliance goals.
- 5 years of pharmacist experience (or 3 years at a specialty pharmacy).
- Experience using software to improve business processes; familiar with MACOS, Google Apps, and database applications.
- Strong interpersonal skills and ability to maintain cooperative working relationships across cross‑functional departments.
- Excellent communication skills and enthusiasm for providing an outstanding patient experience via phone, text, and email.
- Ability to work independently and follow‑through on assignments with minimal supervision.
- Strong working knowledge of applicable state and federal pharmacy laws.
- Ability to review and assess prescription appropriateness for age‑specific and disease‑specific considerations.
- Ability to execute new processes and protocols quickly and accurately, with an interest in defining future processes.
- Specialty Pharmacist certification (e.g., AAHIVP, CSP).
- Completed specialized training or knowledge of specialty drugs/disease states (Biologics, LDDs, HIV/PrEP, MS, Oncology, etc.).
- Pharmacy manager or Pharmacist In‑Charge experience.
- Current pharmacist licensure in multiple states.
Annual salary range: $ – $ USD. No commission or equity opportunities.
