Case Manager - Foster Care Program
The ideal candidate must reside in Coffee County, Tennessee, in the cities of Tullahoma or Manchester.
Greystone Foster Care, in partnership with Universal Health Services, Inc. (UHS), has a long history in providing behavioral health services to at‑risk youth with extensive mental health needs. Greystone Foster Care recruits foster parents and provides training to create a safe, stable, and therapeutic environment for children and youth removed from their homes. Training prepares staff and foster parents to support children and youth through the healing process.

Greystone Foster Care is seeking a dedicated, compassionate Case Manager to join our team. This is a hybrid role with remote work and in‑office requirements, with occasional on‑site reporting. The position is full‑time and may include weekends and evening work.
Job Duties/Responsibilities:
- Comply with state regulatory requirements and Greystone Foster Care plan
- Ensure that all placement documentation is completed according to established timelines
- Complete regular file audits for each child and participate in regular peer file audits to ensure audit readiness
- Assist with completing foster parent reassessments when necessary
- Schedule child/family team meetings at least quarterly and ensure corresponding documentation is completed and submitted
- Visit each assigned child as per policy or as needed
- Ensure annual EPSDT and biannual dental appointments are scheduled and information provided to DCS
- Ensure outpatient mental health assessments are scheduled for all Level 2 youth and participation in recommended services occurs
- Obtain required consents and releases from appointed guardians for medical appointments and medications

Requirements:
- Bachelor’s degree in Social Work or related field;
Master’s degree preferred - One to three years of related experience and/or training, or equivalent combination of education and experience
- Valid state driver’s license
- Automobile insurance with verification of medical liability
