Alignment Health is seeking a bilingual Spanish Health Coach and Educator to join the interdisciplinary Care Anywhere team in Maricopa, Arizona. This field remote role conducts home and virtual visits to build a wellness coaching relationship with patients, caregivers, and families, promoting better health through support and education.
You'll conduct 6-8 home visits daily, collaborate with clinicians, and help patients access community resources while maintaining HIPAA privacy.
