Care Coach 1

The Care Coach 1 assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness states by guiding members and families toward and facilitating interaction with resources appropriate for their care and wellbeing. The role involves meeting members in their location, spending quality time assessing their needs and barriers, and then connecting them with quality services to promote their ultimate well‑being and drive health outcomes.
The Care Coach 1 work assignments are often straightforward and of moderate complexity and will report to the Regional Care Coach Manager.
We are looking for motivated and dynamic case managers who reside in Hillsborough or Hardee counties in Florida. The current starting salary is $60‑65K per year based on qualifications and experience.
Responsibilities- Visit Medicaid members in homes, assisted living facilities, long‑term care facilities, and other care settings – 75‑90% local travel.
- Assess and evaluate members' needs and requirements to establish a member‑specific care plan.
- Ensure members receive services in the least restrictive setting to achieve and/or maintain optimal well‑being.
- Plan and implement interventions to meet those needs.
- Coordinate services, monitor and evaluate the case management plan against the member's personal goals.
- Guide members and families toward resources appropriate for their care.
- Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in the member's care as required by the comprehensive contract.
- Bachelor's degree in health and human services field.
- At least 2 years of related experience.
- Knowledge of Microsoft Office Word and Excel.
- Exceptional verbal, written communication and interpersonal skills.
- Ability to use a variety of electronic information applications and software programs.
- Must be screened for tuberculosis as part of Humana's TB screening program.
- Must have a valid state driver's license and personal vehicle liability insurance meeting state minimum limits.
- Must have a separate, locked room that can be used as a home office to ensure privacy while working.
- Experience with electronic case note documentation in multiple computer applications or systems.
- Prior experience with Medicare and Medicaid recipients.
- Experience with health promotion, coaching, and wellness.
- Knowledge of community health and social service agencies and additional community resources.

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full‑time employment. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job‑related skills, knowledge, experience, education, certifications, and other factors. $53,700 ‑ $72,600 per year.
