Position Summary
The Registered Nurse is responsible for completing assessments on medically complex children who are currently in Foster Care. This role supplies nursing services and quality management of all nursing functions to the agency and provides health care services for all medically specialized children. The Registered Nurse ensures that comprehensive nursing assessments are completed for clients served by the agency, at the request of the agency.
This is a contract position assigned as needed to do clinical assessments at assigned client Foster Family homes. The Registered Nurse has the flexibility to schedule meeting times with the child or children at the Foster Family home based on your availability. These meetings would be set up with the Foster Family.
Responsibilities- Assures the quality of nursing care required by all applicable standards.
- Provide service coordination (case management) to clients receiving services from the agency, including participation in treatment planning meetings, individual supportive therapy, and symptom management, educating, and supporting clients' families, and advocating for clients' rights and preferences.
- Consult with community agencies and families to maintain coordination in the treatment process.
- Follow up on next home visit to see if referrals are made.
- Documents client progress to maintain a permanent record of client activity according to established methods and procedures.
- Participate in organizational meetings and treatment planning review meetings.
- Coordinate consumer care to ensure quality care is consistently provided to consumers, using best practices.
- Produce and submit reports as requested by the established due date.
- Effectively network with other community agency personnel.
- Serve as an Ambassador in representing Family Core in the community.
- Complete all documentation of care and an assessment for each client Foster Family home assigned monthly. The assessment is due to Family Core by the 1st of the following month along with the mileage report and any other documentation.
- Provide possible resources to Case Worker or Foster Parent.
- Notify the Child Welfare Supervisor or Program Director of any medical emergencies.
- Must be licensed as a Registered Nurse in the State of Illinois.
- Minimum of one year experience or special training in the treatment of children and adolescents.
- Must pass required background check with Illinois Department of Children and Family Services (DCFS).
- Must pass background checks with Illinois Department of Public Health (IDPH) Health Care Worker Registry, and Illinois Department of Healthcare and Family Services OIG Provider Sanctions.
- Must be an initiative-taking, results-oriented individual who is customer focused.
- Must possess valid Illinois Driver's License in good standing and proof of valid vehicle insurance.
- Current physical and TB(PPD) test within 30 days of employment.
- Must be CPR and First Aid Certified.
- Signed employee acknowledgement of physical health and infection control responsibilities.
- Manage and maintain confidential information.
- Ability to utilize computer programs including but not limited to Microsoft Word, Excel, and other software products.
- Safe, reliable personal vehicle to be used for Family Core business.
- Ability to be sensitive and understanding of cultural and lifestyle differences of all people regardless of race, religion, sexual orientation, age, or background.
- Frequently move about the office environment, including walking or standing as needed to complete job duties.
- Regular use of hands and fingers to type, handle documents, or operate office technology.
- Ability to operate standard office equipment such as a multi-line phone, computer (single or dual monitors), printer, copier, and fax machine.
- Ability to sit for extended periods and occasionally stoop, bend, or reach overhead during routine office tasks.
- Occasionally lift or move items up to 10 pounds that are part of typical office operations.
- Perform all job requirements with or without reasonable accommodation.
- Work occurs primarily in an office environment. Occasionally exposed to loud noise and outdoor weather conditions.
- May be subject to distraction and interruptions. May be subject to difficult client interactions that involve behavioral and/or emotional challenges.
