Humana is seeking a Behavioral Health Care Manager to telephonically assess, evaluate, and support members challenged by serious mental illness and/or substance use. Our goal is to provide support for mind and body to allow each member to achieve and/or maintain their optimal health.
This remote telephonic role requires identifying and resolving barriers to care, monitoring progress, and coordinating with care teams and providers. You will report to the Manager, Care Management.
