Role Description The Speech Language Pathologist will provide evaluation and treatment services for individuals with communication and swallowing disorders in both outpatient and home-based settings. This is a contract, hybrid role based in Crystal Lake, IL, with a combination of in-clinic visits, occasional home visits, and some work-from-home for documentation and telepractice as appropriate. Day-to-day responsibilities include conducting comprehensive assessments, developing individualized treatment plans, delivering evidence-based speech and language therapy, and monitoring progress toward functional goals.

The Speech Language Pathologist will collaborate with other rehabilitation professionals, educate patients and caregivers on strategies and home programs, and maintain accurate, timely documentation in accordance with regulatory and clinic standards. The role also involves participation in team meetings and ongoing professional development to support high-quality, patient-centered care.

Qualifications

  • Demonstrated proficiency in Speech Therapy and management of Speech and Communication Disorders.
  • Experience assessing and treating Language Disorders across age groups and functional settings.
  • Clinical skills in evaluating and treating Dysphagia, including safe swallowing strategies and interdisciplinary coordination.
  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Current ASHA Certificate of Clinical Competence (CCC-SLP) or eligibility, and state licensure in Illinois (or ability to obtain).
  • Prior experience in outpatient or home health rehabilitation settings preferred.
  • Strong documentation, organization, and time-management skills, including comfort with electronic health records.
  • Ability to work collaboratively in a multidisciplinary team and communicate effectively with patients, families, and colleagues.
  • Commitment to ethical, inclusive, and patient-centered care, with willingness to adapt interventions to diverse backgrounds and needs.
