Wider Circle in Wisconsin is hiring a Care Navigator to join Care Delivery Operations. In this entry-level role, you'll talk with patients by phone (and sometimes in person) to complete health assessments and intake appointments, build trust, and document findings clearly.
You will schedule intake calls, connect patients to resources, and keep accurate records while following HIPAA rules. Training is provided, with a path to become a Care Advocate and CHW training.
