Physical Therapist - Travel

Join a home health team in Mayfield, KY as a traveling Physical Therapist. Provide evaluations and skilled therapy services to patients in their homes across Graves and Calloway counties. This role requires professional licensure and clinical competency in outpatient and home health settings.

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky physical therapy license required to start the assignment.
• Basic Life Support (BLS/CPR) certification required.
• Ability to perform patient evaluations for therapy services in home health settings.
• Willingness and ability to travel within coverage areas including Graves County and Calloway County.
• Minimum education requirement:
  Bachelor’s degree.

• Home health experience preferred.

• Shift hours: 2000–0400.
• On-call/weekend requirement:
  No.
• Mileage reimbursed between visits, from first patient to last patient.
• Coverage areas include Graves County and Calloway County.

Provide comprehensive home-based evaluations and therapy interventions that improve patient mobility, function, and safety.
