The Role

In this role, you will serve as a clinical leader within a high-growth specialty home infusion program
, managing a dedicated panel of complex patients while collaborating across a multidisciplinary care team. This is a non-dispensing, clinically focused position that emphasizes patient outcomes, coordination, and program development.

On a Typical Day, You Will

• Manage a panel of approximately 50 to 60 specialty infusion patients, supporting them from therapy initiation through stabilization
• Provide ongoing clinical monitoring, evaluating therapy effectiveness, side effects, and adherence for high-cost, high-complexity medications
• Conduct proactive patient outreach and education through a remote, telephonic care model, building trusted longitudinal relationships
• Serve as a central liaison between patients, providers, and pharmacy teams, ensuring seamless coordination of care
• Collaborate closely with physicians, clinic pharmacists, and internal teams to adjust therapies and resolve complex clinical scenarios
• Oversee refill coordination and therapy continuity, ensuring safe and timely medication access
• Participate in weekly process improvement and program development initiatives, contributing to workflow design and policy creation
• Support operational planning such as high-value medication ordering and forecasting, ensuring alignment with patient needs
