SCAN Health Plan is seeking a Patient Care Coordinator to support SCAN clinicians and coordinate care for members to keep seniors healthy and independent.
You will work closely with clinicians, coordinate orders and services, arrange transportation, obtain records, and ensure documentation is complete and timely. This role includes field visits in the San Diego area up to 25% and a remote work component, with a focus on compassionate service aligned to SCAN’s mission.
