Day Kimball Health is seeking a Per Visit Occupational Therapist to join our Homecare department in Thompson, Connecticut. The OT will provide home-based skilled therapy, evaluating functional status and creating plans to maximize independence while collaborating with an interdisciplinary team.
Responsibilities include home evaluations, implementing treatment plans, educating patients and families, and maintaining accurate documentation.
