Live Well Homecare is seeking a Field Care Coordinator to assess enrollees, plan care, and advocate for independence. You will visit members in homes and facilities within your territory, coordinating with care teams and community resources.
The role requires a Virginia license (nursing, social work, or human services) and 3+ years of coordination or healthcare experience. MS Office proficiency and reliable transportation are needed. CCM and Medicaid/Medicare experience are a plus.
