POSITION SUMMARY/RESPONSIBILITIES
Counsels and assists patients and their families with personal and environmental difficulties which predispose illnesses or which creates obstacles in receiving maximum quality of care. Acts as a therapeutic agent by participating within a multi-disciplinary health and mental health teams. Facilitates the delivery of services and support is provided efficiently and in a timely manner. Maintains good working, patient/provider/staff relationships. Complies with and enforces University Health policies and procedures.
EDUCATION/EXPERIENCE
Master's degree in Social Work from a school accredited by the Council on Social Work Education is required. One year's post master's degree experience in a health care setting is preferred.
LICENSURE
Current state licensure from the Texas Department of Human Services is required.

The Behavioral Health (BH) Social Worker assists Community First members with their Behavioral Health needs by assessing and identifying unmet needs and coordinating care, utilizing Covered Services and community resources with the goal of optimizing the Member's well-being, independence, and integration in the community. The BH Social Worker collaborates with other teams in Population Health Management (PHM) to ensure that the member’s physical, behavioral, functional, and psychosocial needs and preferences are met.
BH Social Worker ensures adherence to all regulatory and accreditation requirements and follows Community First policies, procedures, and protocols.
