Are you a passionate Licensed Social Worker looking to make a meaningful impact from home? Join a collaborative care management team where you'll help members navigate complex medical, behavioral health, and social needs while improving access to critical resources and support.
What You’ll Do- Partner with Registered Nurse Case Managers to support members through assessment, care planning, and advocacy
- Identify and coordinate community resources related to housing, transportation, financial assistance, safety, and behavioral health needs
- Engage members telephonically and electronically to support positive health and lifestyle outcomes
- Assist members in overcoming barriers to care and accessing appropriate services
- Collaborate with interdisciplinary teams to address medical, psychosocial, and behavioral health needs
- Support a diverse member population across all stages of life
- Master's Degree in Social Work
- Active, unrestricted licensure as one of the following:
- LSW (Licensed Social Worker)
- LMSW (Licensed Master Social Worker)
- LCSW (Licensed Clinical Social Worker)
- LISW (Licensed Independent Social Worker)
- 2+ years of experience in medical social work or case management
- Ability to obtain and maintain multi-state licensure (minimum of 10 states – willing to obtain additional licenses) before completing new hire training (licensure costs paid by the company)
- Fully remote position available anywhere in the U.S.
- Consistent weekday schedule:
Monday–Friday, 8:00 AM–4:30 PM EST - No nights, weekends, holidays, or travel required

Pay Range The typical pay range for this role is: $51,256.00 - $ This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
