Pay Range: $24–$25.50/hour

The base hourly range for this position is $24-$25.50 per hour for employees hired in the Central Valley, CA. The pay range provided in this job posting represents the typical range or starting rate of candidates hired within the Central Valley region. Candidates hired in Southern California or Bay Area locations may have higher starting salaries due to cost of living and market data differences.

Under the direction of the ECM Core Program Manager, the Lead Care Manager (LCM) provides comprehensive, whole-person care management services to Medi-Cal members enrolled in the Enhanced Care Management (ECM) Program. The LCM serves as the primary point of contact for assigned members and their support systems, coordinating care across healthcare, behavioral health, and community providers to address complex medical and social needs.

The LCM collaborates with interdisciplinary care teams, community partners, and service providers to promote access to care, improve outcomes, and support member stability and overall well-being.

• Serve as the primary point of contact for assigned ECM members and their support systems.
• Conduct comprehensive assessments addressing physical health, mental health, substance use, housing stability, and social determinants of health.
• Develop, implement, and monitor individualized Care Plans and Health Action Plans based on member needs.
• Coordinate care across healthcare providers, hospitals, behavioral health services, and community organizations.
• Advocate on behalf of members to ensure access to appropriate services and resources.
• Utilize evidence-based practices, including trauma-informed care, motivational interviewing, and harm reduction techniques when working with members.
• Conduct field-based outreach and engagement activities to support member connection and ongoing participation in services.
• Collaborate with interdisciplinary care teams, community partners, and service providers to support coordinated care delivery.
• Support care transitions, including coordination with hospitals and other care settings to ensure continuity of care.
• Monitor member progress, evaluate outcomes, and update care plans and goals as appropriate.

• Maintain timely, accurate, and audit-ready documentation in accordance with program and regulatory requirements.
• Participate in interdisciplinary care team meetings and community collaboration efforts.

• Bachelor's degree required; however, candidates with a high school diploma (or equivalent) and substantial experience in case management, care coordination, social services, behavioral health, healthcare, or related human services fields may also be considered.
• Minimum three (3) years of relevant experience in care coordination, ECM, behavioral health, or case management
• Experience working with high-risk or underserved populations
• Strong documentation and organizational skills
• Valid California driver’s license and reliable transportation
