The Case Management Coordinator utilizes critical thinking and judgment to collaborate and inform the case management process. The Coordinator facilitates appropriate healthcare outcomes for members by providing assistance with appointment scheduling, identifying and accessing benefits, and educating members through the use of care‑management tools and resources.

• Evaluation of members: conduct comprehensive evaluation of member’s needs/eligibility using care‑management tools, review data, and recommend approaches to case resolution and/or meeting needs.
• Identify high‑risk factors and service needs that may impact member outcomes, and refer to clinical case management or crisis intervention as appropriate.
• Coordinate and implement assigned care‑plan activities and monitor care‑plan progress.
• Enhance medical appropriateness and quality of care: consult with case managers, supervisors, medical directors and other health programs to overcome barriers, present cases at conferences for multidisciplinary review, and achieve optimal outcomes.
• Identify and escalated quality‑of‑care issues through established channels.
• Utilize negotiation skills to secure appropriate options and services necessary to meet member benefits and/or healthcare needs.
• Employ influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes for optimal health.
• Provide coaching, information and support to empower the member to make independent medical and healthy lifestyle choices.
• Engage with colleagues in ongoing team meetings and offer peer mentoring/training.
• Help members actively and knowledgeably participate with their provider in healthcare decision‑making.
• Monitor, evaluate and document care: use case‑management and quality‑management processes in compliance with regulatory and accreditation guidelines and company policies.

• Must live in or near Winnebago County, IL.
• Two years of experience in behavioral health, social services, or an equivalent field focused on program needs.
• Confidence working at home as an independent thinker, using tools to collaborate and connect with teams virtually.
• Reliable transportation and willingness/ability to travel up to 75% of the time within Winnebago County; mileage reimbursed per company policy.
• Excellent analytical and problem‑solving skills.
• Effective communication, organizational, and interpersonal skills.
• Ability to work independently.
• Proficiency with standard corporate software, including MS Word, Excel, Outlook, PowerPoint, and selected proprietary applications.
• Efficient computer skills, including navigation of multiple systems and keyboarding.

• Case management and discharge planning experience.
• Managed care experience.

Must hold a Bachelor’s degree or a non‑licensed master’s level clinician degree in behavioral health or human services (psychology, social work, marriage and family therapy, counseling).

Hours:

40hours (Full‑time)

Pay Range: $21.10 – $44.99 per hour (or base annual salary) depending on experience, education and geography. Additional eligibility for CVS Health bonus, commission or short‑term incentive programs.
