HARRISONBURG ROCKINGHAM CSB in Harrisonburg, VA seeks a Community Case Manager to join the Community Paramedics team. You will provide crisis intervention, assess needs related to mental health, substance use, and chronic disease, and connect clients with home-based resources to reduce EMS calls.
You will collaborate with medical and social service providers, document in the EMR, support families in crisis, and assist in coordinating medications and services. Work five days a week, 8–5.
