Molina Healthcare seeks a Care Manager RN to support AZ Medicaid members recently admitted to a hospital. Handles inpatient-to-discharge coordination, combines phone outreach with in-person meetings, and navigates EMR/HIPAA systems. Travel to local hospitals is required; mileage reimbursed.
Schedule is weekdays with some flexible shifts after six months. Preferred candidates have case management or inpatient hospital experience and behavioral health exposure.
