Daymark Health is seeking a Care Team member to deliver virtual CBT for cancer patients. You will work with Health Partners, Nurses, and Nurse Practitioners to assess needs and craft care plans across the cancer journey.
Responsibilities include documenting interactions in the EHR, participating in weekly rounds, and linking patients to Palliative, Advance, and End-of-Life resources. This is a fully remote role with EST hours for Mon–Fri schedules.
