Summit Home Care and Hospice is searching for an experienced and motivated Scheduling Coordinator to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.
Scheduling Coordinator:Hours:
Monday - Friday; 8:30 AM - 5:00 PM or 9:00 AM - 5:30 PM

Pay Rate: $18.00 - $22.00/Hour
Responsibilities:- Assign clinical staff to care for patients based on patients’ location, diagnosis, and various patient requirements.
- Utilize and maintain the scheduling software program leading to accurate schedules, increased internal and external customer satisfaction and efficient billing practices.
- Maintain shared email inboxes as well as HIPAA compliant texting are used for these requests as well as visit change requests.
- Coordinate and initiate timely communication amongst staff and patients.
- Plot out frequencies as requested from clinical staff via email.
- Update patient charts as needed for hospitalizations, outpatient transfers, or hospice admissions.
- Have adequate understanding of visit timeliness, appropriate medical terminology, home care guidelines, etc.
- Work closely with other internal departments and maintain an understanding of how these departments work together.
- Provide exceptional customer service to patients.
- All other duties as assigned/directed.
Experience:
- High School Diploma or equivalent required.
- 2+ of experience in healthcare experience.
- Prior clinical scheduling experience highly preferred.
- General understanding of medical terminology and HIPAA regulations.
- Ability to work independently or as an active member of a team.
- Strong computer skills in data entry, and Microsoft applications.
- Accurate and precise attention to detail.
- Ability to multitask, prioritize, and manage time efficiently.
- Excellent verbal and written communication skills.
- Goal-oriented, organized team player.
- Must successfully pass a background check in accordance with state and federal regulations.
