We are hiring a Medical Social Worker (MSW) for a Hospice position providing care to patients in their homes in the Kansas City, MO.

- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Conduct comprehensive psychosocial and bereavement assessments, identifying patient and family needs while developing care plans that support emotional, social, and end-of-life goals
- Provide counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey with clarity and dignity
- Apply critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation
- Work independently but never alone

- One (1) to two (2) years post graduate social work experience in a health care setting
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
