Wellspring seeks a Community Living and Supports Resource Coordinator to coordinate HCBS access, implementation, and quality oversight for individuals with DD/SLS/CFC waivers in Colorado. You will collaborate with individuals, families, guardians, CMAs, and providers to promote independence, meaningful community participation, and compliance with Medicaid waivers.
You will monitor service plans, support coordination, and ensure timely documentation while maintaining person-centered outcomes.
