Role Overview
A Full-Time Physical Therapist provides skilled home-based rehabilitation services to help patients recover, improve mobility, and enhance quality of life.
Key ResponsibilitiesDeliver individualized physical therapy plans, evaluate patient needs, set rehabilitation goals, and adjust treatments as needed. Collaborate with interdisciplinary teams, educate patients and caregivers on exercises and safety, and document all visits accurately. Support patient independence and well-being in their home environment.
Qualifications & SkillsMaster’s degree in Physical Therapy, valid IN licensure, CPR certification, strong communication and problem-solving skills, and reliable transportation. Preferred experience includes home health or outpatient rehab, geriatrics, orthopedics, or neurologic rehab.
