Well Be Senior Medical is seeking a licensed clinical social worker (LCSW) to join our home-based geriatric care team in Ohio. You will deliver primary, urgent, and supportive counseling to elderly patients in their homes, coordinate with physicians and care teams, and connect families with community services to improve outcomes.
Candidates should have a Master’s in Social Work and an active LCSW license, with at least five years’ geriatric clinical social work experience.
