Center Well Home Health is seeking a dedicated Physical Therapist to provide home-based therapy, helping patients regain strength, mobility and independence under the supervision of the Clinical Manager.
You will plan and administer prescribed PT, assess patient progress, and collaborate with a care team to meet personalized goals in the patient’s home setting. APTA-approved degree and an active PT license are required; a $20K sign-on bonus is available.
