About the Role

We are seeking a motivated and compassionate Physical Therapist to provide in-home outpatient (OP) therapy services to adults across a variety of clinical presentations. This role is ideal for clinicians who value autonomy, flexibility, and meaningful one-on-one patient care—without the high-volume, VNA-style documentation workload.

You will evaluate, design, and implement individualized treatment plans aimed at improving mobility, relieving pain, enhancing function, and preventing disability. Our model emphasizes quality care, clinical independence, and schedule flexibility.

Why This Role Stands Out

  • Autonomy & Flexibility:
Build your own schedule and manage your caseload.
  • In-Home Outpatient Model:
Deliver outpatient-level care in a comfortable home setting.
  • Reduced Administrative Burden:
Streamlined documentation processes—no VNA-level paperwork requirements.
  • Choose Your Service Area:
Practice within a geographic radius that works for you.
  • Control Your Volume:
Set your preferred number of weekly visits.

Clinical Focus

  • Orthopedic conditions

  • Cardiopulmonary diagnoses
  • Chronic pain management
  • Balance training & fall prevention
  • Geriatric rehabilitation

Experience in home-based adult care is strongly preferred.

Qualifications

  • Graduate of an accredited Physical Therapy program
  • Active Massachusetts PT license in good standing
  • CPR certification
  • Minimum 2 years of clinical experience preferred (geriatric experience strongly preferred)
  • Strong clinical reasoning and manual therapy skills
  • Excellent interpersonal, communication, and patient education abilities

Working Environment

  • Primarily mobile, community-based in-home outpatient services
  • Requires occasional lifting and patient mobility assistance
  • Utilizes Google Workspace for communication and documentation

Benefits

  • 401(k) with 3.5% employer match
  • PTO and sick leave
  • Workers' compensation coverage
  • Mileage reimbursement at current federal rate
  • Continuing education reimbursement up to $500/year
  • Flexible scheduling options

This position is ideal for Physical Therapists seeking professional independence, meaningful patient relationships, and a better work-life balance while maintaining high clinical standards.
