- Maternal Health.
Healthy Families of the Blue Ridge is seeking an experienced and collaborative Senior Community Health Worker to join our Community Health team. This grant-funded role is pivotal in supporting home-visiting staff, conducting comprehensive assessments for program referrals, and expanding partnerships with local agencies and resources to better serve at-risk families in our Healthy Families program. This position serves as a trusted liaison between Sentara Healthcare and pregnant and postpartum individuals within the community.

The role integrates community health outreach with culturally responsive perinatal services to improve maternal and infant health outcomes, reduce disparities, and address social determinants of health. Responsibilities include providing education, advocacy, emotional support, care coordination, and system navigation throughout pregnancy, childbirth, and the postpartum period.
Bilingual candidates are encouraged to apply; proficiency in another language is preferred but not required.
Education- High School Diploma or Equivalent (Required)
- College coursework in child development, public health, human services or other related field. (Preferred)
- Valid Driver's License with an acceptable driving record (Required)
- 3 or more years of relevant experience (preferred)
- Demonstrated leadership skills and experience (preferred)
- Cultural competence and willingness to serve diverse populations (preferred)
- Strong knowledge of community resources, health, and social services (preferred).
- Provide leadership, guidance, and support to home-visiting staff
- Conduct intake and eligibility assessments for program referrals
- Collaborate with referring community organizations, healthcare providers, and agencies to strengthen partnerships and enhance family support networks
- Facilitate ongoing training, peer support, and reflective practice for home-visiting team members
- Serve as a community ambassador for the Healthy Families program
- Maintain accurate documentation of assessments, partnerships, and outreach activities
- Assist with program development and continuous quality improvement initiatives
