Belle, LLC seeks a compassionate Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries in the Chronic Care Management program. You will respond to urgent clinical escalations, assess patients remotely, and collaborate with nurses and primary care providers to keep patients healthy and out of the ED when appropriate.
The role requires an active NP license (GA preferred), national certification, and prior telehealth experience.
