Winding Waters Clinic is looking for a Connected Care Coordinator/Community Health Worker (CHW) to join our mission of providing exceptional, patient-centered care across Wallowa County. In this dynamic and fulfilling role, you will work directly with older adult patients and their families to provide education, home-based assessments, and essential care coordination to help them live safely and thrive in our community.

What You Will Do
  • In-Home Care & Assessment:
    Conduct home visits to complete Connected Care Protocols, assess health and social needs, identify fall hazards, and assist patients with medication management and Advance Directives.
  • Bridging & Coordination:
    Collaborate closely with primary care providers and health care teams to advocate for patients and align care plans with what matters most to them.
  • Outreach &

    Education:

    Deliver culturally competent health education on healthy aging, guide families on available social services, and assist with application processes.
  • Crisis Support & Advocacy:
    Offer emotional support, intervene during health-related challenges, and connect patients to mental health or crisis resources when needed.
  • Closing the Loop:
    Maintain accurate records in our Electronic Health Record (EHR) system, track outcomes, and participate in continuous program evaluation.
Who You Are
  • The Older Adult Advocate:
    You have a genuine passion for working with older adults and a commitment to helping them navigate health and life transitions.
  • The Compassionate Communicator:
    You possess strong interpersonal skills, empathy, and a culturally sensitive approach to supporting diverse populations.
  • The Self-Starter & Team Player:
    You are comfortable working independently in home and community settings while collaborating smoothly with clinic clinicians.
  • The Resource Navigator:
    You understand local healthcare resources, social services, and systems like Medicare and Medicaid (Oregon Health Plan).
Qualifications
  • High school diploma or equivalent.
  • Valid driver's license, insured vehicle, and willingness to travel locally.
  • 90-hour Community Health Worker Certification, or the ability to become certified.
  • Minimum of two years of experience in a health or social service-related field
  • Proficiency in basic data entry, record-keeping, and EHR systems.

Compensation & Benefits
  • Wage:
    Strong guaranteed base wage commensurate with experience and certified skill sets. Competitive wage range of $18.50–$30.00/hour (DOE).
