Community Health Worker Program Manager

Position

Purpose:

The Community Health Worker (CHW) Program Manager is responsible for supporting community health activities by overseeing vendor/partner organization, coordination of patient support with providers and care teams, coordinating activities related to logistics, curriculum, reporting, and analyzing program data. The CHW Program Manager operates within a public health framework emphasizing prevention, health equity, and community-based care integration. This role is accountable for full population management across the CHW-supported population, including engagement, outcomes, and integration with care management and quality.

Under the department HE Director's leadership, the Community Health Worker Program Manager will serve as the primary contact to the specific vendor/partner interacting extensively with internal and external stakeholders, including members. The Community Health Worker Program Manager will also actively develop policies and procedures and execute the specific required tasks for the manager is responsible for the creation and delivery of presentations to internal and external stakeholders surrounding the program contractual compliance, status of patient care, successes in operations and opportunities for improvements.

The role ensures alignment with existing chronic disease infrastructure and positions CHW services as a core component of the overall clinical and population health strategy.

* Travel Requirement:
This is a remote position with regular local travel primarily throughout Passaic County in northern New Jersey. Travel can include to our Iselin, NJ office, provider offices, community-based organizations, partner offices. Candidates residing in New Jersey—ideally within or near Passaic County—are strongly preferred due to the field-based nature of the role and in office meetings. Occasional evening or weekend travel may be required to support community events.

  • Lead population-level strategy for CHW-supported members including engagement and outcomes accountability.
  • Integrate CHW programs into chronic disease management and clinical infrastructure.
  • Monitor outcomes, utilization, and health equity metrics and drive program improvements.
  • Develop and roll out work plan and timelines for program.
  • Schedule weekly meetings to discuss status of program upon execution of Agreement.
  • These weekly meetings will continue for at least six (6) months following program execution, or longer as reasonably requested.
  • Attend on-boarding and file layout review sessions.
  • Communicate progress and make sure the appropriate amount of time for the resources is allocated to meet key milestones on the setup of the data aggregation process (design, development, testing).
  • Attend all state, stakeholder, and community-based organization meetings and is able to work with the dedicated resources at DMAHS to resolve and identify issues/barriers per CHW program requirements.
  • Schedule quarterly business review meetings.
  • Respond to inquiries from the vendor/partner organization as questions arise including but not limited to program design, implementation conditions, budgetary, etc.
  • Handle issue resolution related to adhering to budget, grant management, contractual obligations, inventory levels, Data and service levels covered under this contractual requirements.
  • Respond within one (1) business day explaining how the issue is being addressed.
  • Provide program launch support in partnership with vendor/partner organization.
  • Assist in staff training and in-services for program implementation/initiatives.
