Accompany Health is seeking a Health Advocate to support patients with complex health and social needs through in-home visits, virtual visits, and coordination with an interdisciplinary care team. You’ll help patients advance care plans, track progress, and ensure adherence to care plans.
You will build trust with patients, set up and troubleshoot virtual visits, perform vitals collection, and document observations while collaborating with clinicians and community health workers.
